It also provides an ongoing health management program with direct and continuous access to your primary care team during regular business hours

The membership-based program is an ideal fit for individuals who value having a partner to help navigate their health journey and enjoy a more high-touch service experience.

Concierge Medicine is a subscription-based, primary care program. Concierge Medicine, also known as direct primary care, allows a patient to communicate directly with the concierge team outside of appointment times and at the patient’s own convenience. Where most traditional primary care models require an appointment during office hours to speak with your physician, members of the Concierge Medicine program can call a number that goes straight to the concierge physician’s cell phone. 
Insurance does not provide coverage for the membership fee. These fees grant access to services beyond standard medical care, like extended appointment times, 24/7 access to the doctor, and personalized wellness plans. Flexible Spending Account (FSA) or Health Savings Account (HSA) funds can be used to cover the cost of the program. The membership fee provides direct access to your care team through a variety of contact methods. Rather than being required to schedule an office visit, care can be received through direct phone call, text message, video chat, or in person so that care is convenient for you. While not required, we do encourage patients to maintain healthcare coverage to cover the cost of emergency or specialty care, and referrals from the Concierge Program where insurance may be utilized. When a referral to a specialist, additional labs, or imaging services are deemed medically necessary, our practice will facilitate the referral process, and the services provided at that time will be billed to your health insurance according to your plan's benefits.

The Concierge Medicine program includes primary care services with direct access to a doctor and concierge care team. Access also includes direct correspondence, same day or next day appointments at home, in the office, on the phone or through video. Your concierge provider holds multi-state licensure, ensuring continuity of care for those who spend time out of state. The membership includes annual wellness visits or physicals, some annual screening labs and some discounted lab rates. Membership also includes discounted healthcare and hospitality services through Vail Health partners.
We are dedicated to broad scope primary care. This enables us to manage many conditions directly through your care team. Should you need specialty care, the concierge care team will refer out of the program to trusted experts to provide consultation throughout the healthcare journey.
Our care team is dedicated to offering the highest levels of service and quality care. To do so, we must limit the total number of patients in our program. The first step to entering our program is to complete a brief Interest Form. Interested parties will be provided additional information, and invited to participate in an membership interview. Formal enrollment takes place with the completion of a membership agreement contract and payment of membership fees.
Potential Concierge Medicine patients must complete a contract, sign consent forms and pay for the membership in full. After fully enrolling in the program, each concierge member will receive a card with concierge phone numbers and email addresses. Patients may also correspond via patient portal (EHR email).  

Concierge Medicine is a separate department that is closely associated with traditional primary care services offered through Colorado Mountain Medical. This is because the concierge membership program operates outside of insurance. Currently, the concierge team is comprised of Dr. Melissa O’Meara (Internal Medicine) and Jordan Levy (APRN). When patients join the program, they transition their primary care from their existing provider to the Concierge Medicine department.  

We are dedicated to providing our members with unhurried, convenient care. As much as possible, the direct concierge number will take you straight to your concierge provider and/or care team. At times, this may be a bit delayed if they are with other clients. All medical emergencies should continue to use 911 or other emergency care options.
The concierge membership is a 12 month contract that outlines the roles and responsibilities of both parties. We do not offer partial year contracts at this time. Membership is paid in full at the time contracts are completed and automatically renewed unless the patient opts-out prior to the renewal date.
Concierge members have exclusive access to their care team. Spouses and adult children can join the concierge program at a discounted price.
Annual physical labs are included with the concierge membership. The concierge department will have exclusive access to discounted rates for some advanced labs and imaging studies. Your care team will personalize your health plan to meet your specific needs and goals. Should you need laboratory or imaging beyond the annual physical, then this will be ordered by your concierge provider and sent through your insurance when applicable or additional cash pay services may be available at a wholesale rate through exclusive relationships within the Vail Health Concierge Medicine Program.
A concierge physician has a limited panel of patients. On average, a concierge physician will care for 400 patients per year whereas most traditional primary care providers see 2,000+ patients per year. Because of the limited panel size, concierge providers have room in their schedule to offer same-day appointments to their active members.  
Concierge members have several options when it comes to care delivery. They can talk directly to their doctor via text, telephone, or through video visit. They can come to our offices in Avon (50 Buck Creek Rd, Suite 300) for an office visit. They can also request a home visit from their concierge care team.  
Vail Health is responding to requests from the community. For many years, members of the community have requested a program that allows direct access to their primary care team. Some locals participate in concierge programs in Denver and have requested that we provide services closer to home.
