Through this portal, you can complete paperwork associated with your upcoming care, all at your own convenience from your own computer or mobile device. Completing the registration process online will save time when you check-in for your appointment.
Below are some frequently asked questions regarding the registration and check-in process
Why are you asking me to pre-register for my appointment?
Our goal is to make the day of your appointment as easy as possible. That starts with your arrival at the hospital and the check-in process. We have created this online portal to allow you to fill out your registration paperwork ahead of your appointment. By taking a few minutes to complete this now, you’ll be able to expedite your check-in the day of your appointment.
How long will it take to pre-register?
Online registration typically takes only 10-15 minutes and, when finished you will receive a barcode that you can scan at one of the on-site kiosks or tablets located inside the main entrance. This will allow for a faster, easier check-in experience when you arrive for your appointment.
How does it work?
Every patient that has an imaging or surgical appointment has the chance to fill out their registration information ahead of time. If you received an email confirming your appointment then you can take advantage of this process! The process goes like this:
- You will receive an email inviting you to complete pre-registration after your appointment has been scheduled.
- Click on the link in your email to set up an account in our Appointment Pre-Registration Portal.
- Select your upcoming appointment from your “To Do” list
- Fill out your patient information (home & mailing address, insurance information, language preferences, etc.) and fill out any forms that are provided based on your appointment type.
- Once you have finished registering you’ll be provided a confirmation message that contains a barcode. This barcode is what you’ll use to scan yourself when you arrive at the hospital. If you don’t want to print it out that’s okay, we’ll email it to you as well.
- Arrive at the hospital as instructed by your surgeon or imaging instructions indicate and make your way to the kiosk located inside the main entrance of the hospital. Our main entrance is located at 180 South Frontage Road West.
- Once at the kiosk you can scan your barcode to check in.
I can’t remember my username. How can I get that?
If you can’t recall your username please click on the “Retrieve Username” link on the right side of the screen, above the “Username” box. This will walk you through the process of retrieving your username.
I can’t remember my password. How can I get that?
It happens to the best of us. Once on the login screen please click on the “Forgot My Password” link on the right side of the screen, above the “Password” box. This will walk you through the process of resetting a new password.
How do I change my appointment?
If something has come up and you need to cancel or reschedule your appointment please call us at 970-476-2451.
What is the difference between “Next of Kin” and “Emergency Contact”?
Next of kin is the person with you on the day of your surgical procedure. An emergency contact is your relative or friend listed in our system in case of an emergency
I’m having trouble uploading my photo ID and/or insurance card, is this necessary?
This is required for registration but if you are unable to get it to work you can always do it the morning of your appointment. If you want to do it from the portal you might try to resize the photo and upload it again.
Some of my personal information is incorrect. How do I update it in the portal?
After you sign into your portal account you will see your name in the top left part of the portal. Select the down arrow and you’ll see a link for your “Profile.” Once you are on your profile you can select the “edit” button next to any information you want to update.
I need to stop in the middle of my registration. Will I need to do everything again?
Nope! There is an autosave that continually saves your work as you fill in the information fields. You can come back anytime to finish up at your convenience.
It’s asking me for my insurance information. Where do I find that?
This information is listed on your insurance card. As long as you are able to upload an image of your card, this information will be received in our system. To upload an image please take a picture of it with your camera phone and send it to yourself. From there you should be able to upload it. Please be sure to get the front and back!
I didn’t get an email with my barcode after I completed registration. What can I do?
If you have already registered but you didn’t receive the email then you can print the barcode from the portal registration page by going to your dashboard and clicking on your appointment. Or, upon arrival, you can go up to any of the kiosks and enter in your name and date of birth to check-in.
How do I get a copy of something I signed on the patient portal?
When you arrive at the hospital you can ask any one of our registration staff members and they’ll be able to get a copy of that document for you!
