Your Discharge from the Hospital
Hospital discharge marks the transition from hospital care to recovery at home or another facility. You may not be fully healed, but you are no longer required to stay in the hospital for medical attention. To ensure a comfortable and safe recovery and reduce the likelihood of needing to return to the hospital, it’s important to be informed and prepared for your discharge process.
Key Points to Know About Your Discharge
- Plan Ahead: Preparation is key. Before discharge day, make arrangements for transportation and any assistance you may need. This allows you to focus on your discharge instructions and final preparations for leaving the hospital.
- Discharge Timing: Routine discharge time is 11:00 AM on your last day at the hospital. Confirm your discharge date and time with your healthcare team.
- Transportation: Ensure someone is available to pick you up by your discharge time. The driver can meet you at the main entrance of the building, located at 180 South Frontage Road West, Vail, CO 81657.
- Prepare Questions: Use a notebook or your phone to jot down any questions you have for your healthcare team. Be ready to take notes during your discharge meeting.
- Assistance with Planning: If you need help arranging transport or have special needs, ask your nurse to contact Social Work or Discharge Planning. This service is available Monday to Friday, 8:00 AM to 4:30 PM.
Post-Discharge Care
After you leave the hospital, be sure to follow the care instructions provided by your healthcare team.
If you experience any surgery-related issues after discharge, contact your physician immediately for guidance.
