Emergency Department Medical Reconciliation Tech

Pay Grade: Min: $18.01 Mid: $22.52. Pay is based upon relevant education and experience.

POSITION PURPOSE: 

Obtain and document in the EMR a patient’s best possible medication history and preferred pharmacy for emergency department (ED) patients. This may include interviewing the patient or caregiver or calling outpatient pharmacies and verifying accurately medication allergies and intolerances. Perform a variety of jobs as requested by clinical leaders including: performing clerical and secretarial duties, working cooperatively with the care team, supporting patient care and established hospital policies and procedures.

ESSENTIAL JOB FUNCTIONS:

  1. Interviewing patients/caregivers to obtain an accurate medication history.
  2. Calling outpatient retail pharmacies to supplement the information obtained through the interview.
  3. Provides confidential administrative support to the department and other areas as required.
  4. Demonstrates team work and strong, positive interpersonal skills. Develops professional and trust-based relationships.
  5. Actively participates as a collaborative member of the team by communicating constructively, identifying interdepartmental problems and resolving conflicts appropriately. Assists the ED staff with various tasks as needed.
  6. Organizes and prioritizes multiple simultaneous tasks.
  7. Coordinating patient and visitor flow safely, while demonstrating good customer service, and displays cultural competency.
  8. Protects patients and employees by assisting in maintaining a clean, safe working environment and adhering to infection control policies and protocols.
  9. Role models the principles of a Just Culture.
  10. Perform other duties as assigned. Must be HIPAA compliant.

Experience:

  • Two years pharmacy technician experience preferred; required if not CPhT

Certification(s):

  • State of Colorado Pharmacy Technician Certification (PHAT) required
  • Basic Life Support (BLS) by the American Heart Association (AHA) required
